Cascade Family Medical Clinic is interested in the financial as well as the medical well-being of our patients.  In an effort to keep our patients informed and to expedite the payment of insurance and private pay claims, the following procedures have been implemented.

Upon arrival for every appointment, please present your insurance card and your copay, if applicable.  If we need to bill for a copay, there will be an additional service charge of $5.00 added to your bill.  We do accept VISA and Mastercard credit/debit cards.

You will be asked to sign a visit slip stating you agree to be responsible for any charges your insurance does not cover.  You may also be asked to update your patient registration and insurance eligiblity forms.  Patients having physicals will be asked to sign a Routine Preventative Care form.  Because legal documents are being signed, we ask that children under the age of 18 be accompanied by a parent or legal guardian.

Once payment has been received from the insurance company, a statement of the balance will be sent to the patient.  Payment in full is expected by the first working day of the following month.  If you cannot make a full payment, please contact our billing service (1-866-918-0119) to make payment arrangements.  For proper crediting of your account, please place the account number on all checks and correspondence.

If you do not have medical insurance coverage, we ask that you pay cash at the time of the visit or make payment arrangements with our billing service prior to the visit.  A 20% discount will be applied to all non-insured charges that are paid in full at the time of service.

CFMC will bill some third party insurance companies.  Please provide a copy of any insurance cards and paperwork upon arrival.  You will also be asked to complete a short questionnaire.  The patient is still responsible for the charges if the claim is not paid in full within an appropriate length of time.
