Beginning September 30, 2019 and for the month of October, 2019, medical records requests can not be accepted at Cascade Family.  During this time, patient medical records will be transfered to a records retention company.  Requests will again be accepted beginning in November.  At this time, requests can be made through a link that will be posted on this website. 

Requests previously submitted will be processed at Cascade and patients will be notified when their records are available for pick up.  Records can no longer be faxed to your new provider.  If you would like your records mailed, a prepaid postage fee is required.

The office will remain open Mondays, Tuesdays, & Thursdays from 10:00am to 2:00pm through October 24, 2019.  Thank you.
