Our providers share the concern of their patients about the increasing cost of providing medical care. Our fees are the usual and customary charges for this area. Copays are due at the time of service. There is a $5.00 Late Copay Fee if we are unable to collect a known copay at the time of service and must bill for it.
We understand it is difficult to budget for health emergencies and will work with you to set up a payment plan. However, we are not in the business of extending credit and request that you pay in full within 60 days. Patients without insurance who cash pay in full at the time of service, will receive a 20 percent discount.
CFMC will bill some third party insurance companies. Please provide a copy of any insurance cards and paperwork upon arrival. You will also be asked to complete a short questionnaire. The patient will still be responsible for the charges if the claim is not paid in full within an approprite length of time.
